To qualify for breast reduction surgery through the NHS one must be referred by their GP to a specialist, have a consultation with a plastic surgeon and be assessed by a psychiatrist or psychologist so that they can review your individual circumstances and work out the best option for you. If the conclusion is that your physical or psychological well-being are being affected by the size of your boobs and that breast reduction would help you then they will usually go ahead with the reduction mammoplasty through the NHS.
